Alternate Format Text Agreement
- I certify that I have been approved for AFT as an accommodation and enrolled at Rutgers University for the semester in which I am requesting the following course materials in alternate format.
- I certify that the course materials I am requesting are required for a course in which I am currently registered and is required for the completion of the course.
- I certify that I have read the material from “How to Find your Textbook in Electronic Formats” and the course materials I am requesting are not available to me in an accessible format through other resources such as Bookshare, any Rutgers library or e-reserves.
- I understand that I must provide Rutgers Access and Disability Resources (RADR) proof that I have access to the print version of the materials, either through the Rutgers Library or e-reserves or that I have purchased or rented the print version of the course material. This is required by copyright laws.
- I understand the University will not produce materials that are directly available to me (with or without purchase) in an alternate format that meets my needs.
- I understand that if RADR is unable to obtain an accessible copy of requested materials, I will need to provide my physical copy of such material to be scanned. I understand that the scanning process may require the removal of the material’s binding and cutting of pages to be scanned. RADR will provide the book back with a spiral binding.
- I agree to use the course material in Alternate Format/Electronic File solely for my own educational purposes.
- I will not copy, duplicate, or distribute the course material in Alternate Format for use by others for monetary gain or any other purposes in accordance with US Copyright Law.
- I understand that AFT requests can take up to two weeks to be delivered, but RADR will do their best to deliver them as quickly as possible.
- I certify that I have read and understood all of the above, and understand that failure to follow this agreement may constitute a breach of University Code of Conduct.
